Iron deficiency: oral iron or IV iron? Questions to ask your doctor
A balanced overview of how iron deficiency is treated, why some people are offered an iron infusion, and what to ask before choosing.
First, confirm the cause
Low haemoglobin does not always mean low iron. Doctors usually check ferritin and other iron tests, and look for the reason iron is low. Finding and treating the cause, such as slow blood loss or poor absorption, matters as much as replacing iron.
Oral iron
Iron tablets or syrup are the usual first choice for many people. They are widely available and low in cost. Possible downsides are stomach upset, constipation, dark stools and the fact that absorption can be poor in some situations. Many people need to take them for several months, because the body needs time to rebuild its stores.
IV iron
With IV iron, iron is given through a drip into a vein under medical supervision. A doctor may consider it when:
- tablets cause side effects that you cannot tolerate,
- the gut does not absorb iron well,
- ongoing blood loss is faster than tablets can replace, or
- iron is needed more quickly.
IV iron is not for everyone, and it is given where staff can monitor for reactions.
Questions to ask your doctor
- What is the cause of my low iron, and has it been looked for?
- Why is oral iron or IV iron better for me?
- How long will treatment take, and when will blood tests be repeated?
- What side effects should I watch for?
- What does the fee cover? At Heamcare, the published fee is for administration only; medicines and separately agreed charges are extra.
Urgent symptoms
During or after any treatment, difficulty breathing, swelling of the face or throat, a widespread rash or feeling faint needs urgent medical help.
You can read how a session works in what happens in an IV iron session, or see the IV iron request page. Treatment requests are always reviewed by the doctor first.
Frequently asked questions
Is IV iron stronger than tablets?
Both aim to restore iron. IV iron delivers iron directly into the bloodstream, so it can be useful when tablets are not tolerated, not absorbed well or not working. Whether it suits you is a doctor's decision.
Can I choose IV iron myself?
Treatment is decided after the doctor reviews your history and test results. At Heamcare, IV iron is a request that the doctor reviews and approves before payment.
